SUBMIT PARENTAL PERMISSION

I give permission for my child (or children) to stay at the Portsea SLSC Clubhouse. I understand that my child will be staying at the Portsea SLSC Clubhouse with other active members of the Club (who may be over 18) and that the only supervision will be from the supervisor stated above. I understand that I must be available to pick my child up if required. I understand that there is a licensed restaurant and bar running at the Clubhouse, and that my child must not be in the licensed area after 10pm.
